Narratives of One Health in Action One Health can provide a safe, no judgment zone for many needed conversations surrounding our most difficult global and environmental health challenges. ‘Narratives of One Health in Action’ is a page where many items can be shared, from peer reviewed scientific case studies to examples of One Health successes (or missed opportunities) to Op Eds and Opinion Pieces. Opinions expressed here are not necessarily approved by the One Health Commission but are shared ‘because’ this is a safe, no judgement zone and we need to hear from many perspectives on many of today’s challenges. Historic Physician Epidemiology Leader and One Health Advocate Dies D. A. Henderson, MD, MPH https://en.wikipedia.org/wiki/Donald_Henderson, the great American physician who directed the eradication of small pox in the 20th century and a strong supporter/advocate of the One Health concept, died August 19, 2016. Dr. Henderson served on the One Health Initiative Autonomous pro bono team’s (OHI) Advisory Board http://www.onehealthinitiative.com/advBoard.php. Early in the development of the OHI’s campaign to promote the One Health approach to worldwide public health and clinical health (comparative medicine), Henderson expressed strong support and encouragement. April 22, 2007 Dr. D. A. Henderson, legendary leader of the worldwide smallpox eradication program said: “I thank you for your email and congratulate you and your colleagues in promoting the One Medicine [now generally referred to as One Health] concept. It is an initiative that is long overdue but, at the same time, I dont personally identify dramatic solutions that are apt to change the landscape in the short term. I would note that when one has had the good fortune to have enjoyed the tutelage of *Jim Steele during my tenure at CDC and periodically ever since, as a friend, the one medicine concept becomes well engrained. Indeed, when I came to Hopkins as Dean in 1977, I cast about to determine how we might link up with a veterinary school for research and educational purposes. The OIE (World Organization for Animal Health) introduces its new ‘One health’ web portal “A large amount of information on the ‘One Health’ concept can be found on this platform, along with details of global level action taken by the OIE and its partners to promote cooperation between the human health and animal health sectors.” Paris, 12 August 2016 - Introduced at the beginning of the millennium, this concept expresses in a few words the acknowledgement that human health and animal health are interdependent and related to the ecosystems in which they coexist. Diseases of animal origin which can be transmitted to humans, such as avian influenza, rabies, Rift Valley fever and brucellosis, pose global threats to public health. Other diseases which are mainly transmitted from person to person also circulate in animals or have a known animal reservoir, and are susceptible to causing serious health crises, as the recent epidemic of Ebola virus demonstrated. These risks grow stronger with globalisation, climate change and changes in human behaviour, all of which provide opportunities for pathogens to colonise new areas and evolve into new forms. The OIE believes that this concept requires a global collaborative approach to tackle the risks to human, animal and environmental health as a whole, and takes action accordingly. It builds on its intergovernmental standards and the worldwide information on animal health that it gathers, as well as its network of international experts and programmes for strengthening the capacities of national Veterinary Services. Moreover, it works in collaboration with more than 70 other international organisations, in particular those that play a key role at the human–animal–environmental interface. The new web portal brings together the tools developed by the OIE to control worldwide animal health risks more effectively, presenting the actions taken with its partner organisations – in particular, the WHO and the FAO, within the framework of the Tripartite Alliance – as well as the tools used by stakeholders on the ground to support the joint work of the human health and animal health services. Call for Information on One Health Research and One Health training and education activities in Western Europe We are asking for your help to complete an overview of previous, current, and planned One Health (OH) research and training activities, including antibiotic resistance (ABR) in Western Europe (Austria, Belgium, France, Germany, Italy Iceland, Ireland, Liechtenstein, Luxembourg, Monaco, Netherlands, Portugal , Scandinavia, Spain, Switzerland, UK). This overview is to be published as a review in a special issue of OH review articles for Current Clinical Microbiology Reports and will focus on One Health in infectious disease research and training. We designed a short and simple questionnaire [see Attached PDF], that we hope you can complete to aid us in compiling a complete overview of relevant OH activities in infectious disease research and training/education. It is a very nice opportunity to promote the One Health concept and any One Health activities that you may be involved in that are currently in place or that are planned for the future. It is a worldwide strategy for expanding interdisciplinary collaborations and communications in all aspects of health care for humans, animals and the environment in an effort to improve the health of all species. The One Health Initiative (OHI) is focused on educating international multidisciplinary scientific communities, political and governmental leaders, the news media, and the general public about the One Health concept. This has been achieved via numerous national and international publications, book chapters, presentations, and articles. The OHI team works collaboratively with many national and international organizations, such as the American Association of Public Health Physicians and the One Health Commission (OHC). The OHI team currently participates in all known international One Health Journal endeavors. In addition, the OHI team has worked closely with the One Health Newsletter, a widely read online One Health product of the University of Florida (Emerging Pathogens Institute-USA) --formerly produced by the Florida State Department of Health (USA). The OHI autonomous team was co-founded in 2006 by physician Laura H. Kahn, MD, MPH, MPP and veterinarian Bruce Kaplan, DVM. Thomas P. Monath, MD joined one year later. Jack Woodall, PhD, health research scientist joined in 2009, and public health veterinarian Lisa A. Conti, DVM, MPH joined in 2012. Two years later, Drs. Kahn, Kaplan and Monath initiated the OHI website on October 1, 2008 and Dr. Kaplan is the primary OHI website contents manager/editor. The policy of the OHI team is to provide free access for all news, information, and events pertaining to One Health. All reputable One Health organizations, and individuals in the US and worldwide are recognized and welcomed as supporters and advocates without discrimination. The OHI team works pro bono and does not require nor accept fees from any One Health supporter or advocate organizations and who wishes to participate. The OHI website has been referred to internationally by many as the clearinghouse for all pertinent One Health information worldwide and recently some prominent One Health advocates have called it “the New York Times” of One Health. The OHI website now has at least 118 reciprocal links from within the US and internationally. Yellow fever: World Health Organization (WHO) authorizes emergency expansion of global vaccine stocks by using fractionated doses WHO has published a statement endorsing the emergency use of fractionated doses of the vaccine to stretch global stockpile which is being rapidly depleted with requests from at-risk countries not only in Africa but also in Latin America. “1. Preamble This document represents the World Health Organization (WHO) Secretariat position on the use of yellow fever (YF) vaccine in the context of supply shortages in response to the current outbreak in Africa in 2016. The development of this paper was led by the WHO Initiative for Vaccine Research with contributions to specific sections from the WHO Departments of Pandemic and Epidemic Diseases, Essential Medicines, and Immunization Vaccines and Biologicals. The evidence and the proposed recommendations, reflected in this document, has been discussed with YF experts and reviewed by the WHO Strategic Advisory Group of Experts (SAGE) on Immunization. SAGE and the YF experts provided input to this paper. The recommendations were vetted by SAGE, but they don’t represent a formal SAGE recommendation. The paper will be updated as additional data become available. A full review on the use of fractional dose YF vaccine will be conducted by SAGE in October 2016. 2. Introduction Ongoing YF outbreaks are sharply increasing the demand for YF vaccine, exhausting the global stockpile and putting at risk the immunization of endemic populations. The campaigns currently planned have led to a shortage of the vaccine, a situation which could deteriorate further should expansion of outbreaks necessitate additional immunization campaigns on a large scale. An assessment of existing opportunities to increase the availability of vaccine in response to ongoing outbreaks is therefore urgently required. This paper reviews the evidence on dose-sparing strategies through fractional dosing of YF vaccine as an immediate and short-term option to meet the needs of large-scale campaigns, and proposes recommendations for fractional dose vaccination in case of imminent need in the context of outbreak response. The paper is intended to support efforts to introduce YF vaccine fractional dose use in situations where supply capacity is threatened or inadequate, e.g. following the spread of YF into densely populated areas. Massive amounts of human and animal waste applied to agricultural fields alter the global “resistome.” Wildlife never exposed to antibiotics harbor antibiotic resistant bacteria in their feces. The Human Microbiome project has found that microbial cells outnumber human cells by an estimated 10-fold, fundamentally changing our perceptions about health and disease. Taken together, the findings of antibiotic resistance in external and internal environments suggest that human antibiotic use has altered global microbial populations in ways that we do not fully understand. We are fast approaching a post antibiotic era. This presents challenges and opportunities and will require a One Health approach to succeed. Learning Objectives: 1. Understand the 21st century challenges posed by the growing human population and its increasing demands for terrestrial and aquatic animal proteins. 2. Develop familiarity with metagenomics and “The Global Resistome.” 3. Understand how open defecation jeopardizes human, animal, and environmental health. 4.
